Program Director Job Summary We are seeking a compassionate and experienced Registered Nurse (RN) to join our multidisciplinary care team serving frail older adults. The RN is responsible for providing skilled nursing care, conducting participant assessments, coordinating care plans, medication management, health education, and ensuring compliance with all applicable healthcare regulations and safety standards. Responsibilities Responsibilities include, but are not limited to: Conduct nursing assessments and reassessments to identify participant needs and develop individualized care plans with short- and long-term goals. Assess participants' personal care needs and supervise assistance with activities of daily living (ADLs). Document skilled nursing services, progress notes, and participant care accurately and timely. Provide supportive and restorative nursing services in accordance with individualized care plans. Develop and oversee continence training programs. Administer, monitor, and document prescribed medications in compliance with regulatory requirements. Observe and monitor participants' overall health conditions and report significant changes. Serve as liaison between participants and their primary care physicians to ensure continuity of care. Educate staff on disease processes, participant care, infection control, and safety procedures. Oversee medication management, storage, documentation, and compliance with regulatory standards. Train participants in self-administration of medications when appropriate. Collaborate with physical and occupational therapists to supervise therapy maintenance programs. Provide health education and counseling to participants and family members. Assist participants in accessing medical services and community healthcare resources. Support and supervise nursing students as assigned. Maintain compliance with all Center policies, licensing regulations, infection control standards, fire safety, and disaster preparedness requirements. Attend required orientation and staff training programs. Qualifications Current and active Registered Nurse (RN) license issued by the California Board of Registered Nursing . Minimum of one (1) year of experience in a long-term care facility, home health agency, adult day health center, or similar program serving older adults. Public health or geriatric care experience preferred. Strong clinical assessment, documentation, communication, and organizational skills. Ability to work effectively within a multidisciplinary team environment. Requirements Must provide a physician's health clearance completed within the past 12 months confirming ability to perform job duties. Must provide proof of negative tuberculosis (TB) test completed within the past 12 months. Ability to communicate effectively with Chinese-speaking participants and families preferred. Bilingual in English and Chinese (Mandarin and/or Cantonese) preferred to support participant communication and care coordination. Cantonese language skills are a plus. Current CPR and First Aid certification, or ability to obtain within the first 6 months of employment. Must comply with all adult abuse reporting requirements and healthcare regulations..
