You'll follow individualized care plans, administer treatments, and collaborate with the interdisciplinary team to ensure comfort and dignity for every patient.
Key Responsibilities:
Implement nursing care based on comprehensive assessments and care plans Accurately document patient services to reflect the plan of care Educate patients and families on diagnoses and self-care plans Coordinate hospice services with the interdisciplinary team Use equipment and supplies effectively and efficiently Provide pain and symptom management, evaluating patient response Participate in quality improvement and professional development activities
Qualifications:
Valid California LVN license Minimum 1 year of hospice experience Experience with Home Care Home Base (HCHB) preferred Current CPR certification Valid driver's license and insured, reliable vehicle About Seaport Hospice Seaport Hospice is part of the Pennant Group , a nationwide network with over 300 locations.
