Free parking on first day; assignment parking details provided during orientation Responsibilities Coordinate and manage patient care plans across the continuum of care. Collaborate with physicians, nurses, social workers, and interdisciplinary teams. Perform utilization reviews and discharge planning. Assess patient needs and facilitate appropriate services and resources. Ensure accurate and timely documentation in Epic. Advocate for patients to achieve optimal health outcomes. Requirements Active RN License. Minimum 2 years of Case Management experience required . Acute Care experience required . Strong communication, organizational, and care coordination skills.
