Requirements:
2-year acute Hospital Case Management experience performing Care Coordination and discharge planning. Epic Charting system required. Required to work every other weekend.
Licenses/Certifications:
CA State License BLS (AHA)

CA State License BLS Good to haves: 2-year acute Hospital Case Management experience performing Care Coordination and discharge planning CA State License Epic Charting System.
