Full Job Description Salary Range:
$61.30 - $82.75 Nurse Navigator provides comprehensive clinical support, coordination, education, and advocacy for patients across the continuum of care. Working closely with physicians, APPs, nursing staff, and multidisciplinary teams, the Nurse Navigator serves as a clinical resource and primary point of contact to support patients through diagnosis, treatment, and symptom management. Responsibilities include patient assessment, triage, treatment coordination, clinical documentation, patient education and advocacy, care planning, and facilitation of resources. The role utilizes nursing best practices to ensure high-quality, patient-centered care. The Navigator also participates in quality improvement initiatives, protocol development, annual competencies, EMR management, compliance reviews, and training of new staff.
