  • 34/hour BOE & Depending on License Orchard Post Acute is seeking a compassionate and detail-oriented Admissions Nurse (LVN/RN) to join our team.

Essential Duties & Responsibilities Coordinate and facilitate resident admissions from hospitals and community referrals Conduct admission assessments and review clinical documentation for accuracy and appropriateness Communicate with hospitals, physicians, case managers, residents, and families regarding admissions and care needs Verify orders, medications, and required documentation prior to admission Collaborate with interdisciplinary team members to ensure continuity of care Ensure admissions are completed in compliance with federal, state, and facility guidelines Assist with room setup and resident orientation upon arrival Accurately document admissions information in PointClickCare and other systems Participate in care planning and discharge coordination as needed Maintain positive relationships with referral partners and healthcare providers Support marketing and census development efforts through exceptional customer service Qualifications Education & Experience Previous skilled nursing or long-term care experience preferred Admissions or case management experience preferred Experience with PointClickCare (PCC) preferred Licensure & Certifications Current California LVN or RN license in good standing Current CPR/BLS certification preferred Skills & Abilities Strong communication and organizational skills Ability to multitask in a fast-paced environment Excellent customer service and interpersonal abilities Strong clinical assessment and documentation skills Work Environment Skilled nursing facility setting Frequent interaction with residents, families, hospitals, and referral sources Fast-paced admissions and clinical environment We are an equal opportunity employer.
