The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role involves collaborating with healthcare teams, educating patients and families, and managing various health conditions while promoting independence and quality of life. Duties Conduct comprehensive assessments of patients' health status and needs in a home care setting. Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and healthcare providers. Administer medications and treatments as prescribed, ensuring adherence to safety protocols. Provide education and support to patients and families regarding medical conditions, treatment plans, and home care practices. Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes. Maintain accurate documentation of patient care activities in compliance with regulatory standards. Collaborate with interdisciplinary teams to coordinate care for patients with complex needs. Skills Proficient in medical terminology and triage processes to effectively assess patient conditions. Experience working with individuals with developmental disabilities is highly valued. Knowledge of sterile processing techniques to ensure safe handling of medical equipment. Ability to provide education on basic life support techniques to patients and caregivers. Familiarity with home care practices, particularly in pediatric settings, is a plus. Experience or training in administering dermal fillers is advantageous but not required. Strong communication skills for effective interaction with patients, families, and healthcare professionals.
