Duties Deliver expert nursing care to patients in sub-acute settings, including post-surgical recovery, complex wound management, and chronic disease stabilization Administer medications, including IV infusions and injections, following strict aseptic techniques and safety protocols Conduct thorough patient assessments, vital signs monitoring, and physical examinations to evaluate progress and identify emerging needs Coordinate with physicians and interdisciplinary teams on treatment plans, including specialized procedures such as catheterizations, suturing, and tube feeding Utilize electronic health record (EHR) systems like Epic or Cerner for accurate documentation of patient care, medication administration, and medical records Implement infection control practices aligned with CDC guidelines to ensure a safe environment for patients and staff Assist with discharge planning, patient education on medication management, and post-care instructions to promote optimal recovery at home or in assisted living facilities Experience Proven experience in sub-acute care settings such as hospitals, nursing homes, or rehabilitation centers with familiarity in medical-surgical nursing practices Strong background in critical care environments including ICU or Level I trauma center experience is highly desirable Knowledge of complex medical procedures like cardiology interventions, dialysis support, trauma medicine, and infectious disease care Skilled in working with diverse patient populations including geriatrics, pediatrics, individuals with developmental disabilities, memory care needs, and hospice patients Proficiency with EMR/EHR systems such as Epic or Cerner; experience with ICD coding (ICD-10/ICD-9), CPT coding, and medical documentation standards is preferred Demonstrated ability to perform advanced skills such as ventilator management, sterile processing techniques, phlebotomy, and laboratory specimen collection Critical thinking skills combined with excellent communication abilities to collaborate effectively within multidisciplinary teams and provide compassionate patient-centered care Join us in making a meaningful difference by providing expert nursing support in a vibrant sub-acute setting.
