Anchor Health is seeking an After-Hours Hospice LVN/ RN to provide skilled, on-call nursing care outside of standard business hours. Position Overview The After-Hours Hospice LVN/RN provides professional nursing care to hospice patients during evenings, nights, and weekends. This role ensures continuity of care, symptom management, and emotional support for patients and families after 5:00 PM. The nurse works independently while maintaining close communication with the interdisciplinary team, physicians, and leadership to support individualized plans of care. Schedule 5pm-8am Weekends and holidays as assigned On-call/after-hours coverage Key Responsibilities Respond to after-hours patient calls and urgent care needs Perform skilled nursing assessments and symptom management Provide emotional support and education to patients and families Implement and update patient plans of care as needed Communicate patient condition changes to the interdisciplinary team Collaborate with physicians and caregivers regarding patient needs Complete accurate and timely clinical documentation Uphold hospice philosophy, policies, and regulatory standards Qualifications & Requirements Current RN or LVN license in good standing Hospice or home health experience preferred Strong clinical judgment and ability to work independently Excellent communication and problem-solving skills Compassionate presence in emotionally sensitive situations Valid driver's license Reliable, insured vehicle in good working condition (per state and organizational requirements) What We Offer Competitive compensation Comprehensive benefits package Supportive leadership and team environment Meaningful, patient-centered work A culture that values and respects nurses
