Join Our Team! Are you a compassionate and dedicated LVN looking to provide meaningful one-on-one care in a home health setting? We are seeking a reliable nurse to join our team for a consistent weekday evening case in Hawthorne. This is a great opportunity to build rapport with a long-term patient while providing skilled, specialized care in a supportive environment. Patient Overview We are seeking an LVN for a home health case involving a 25-year-old adult patient with a G-tube (Gastrostomy Tube). The patient requires ongoing skilled nursing support and monitoring in the home setting. Responsibilities Provide skilled nursing care in the home environment Manage and monitor G-tube care and feedings Perform routine patient assessments and documentation Follow the physician's plan of care Monitor patient condition and report any changes promptly Ensure patient safety and comfort at all times Complete accurate and timely electronic charting Communicate effectively with the care team and family as needed Qualifications Active California LVN Current CPR certification Experience with G-tube care preferred Strong clinical assessment and documentation skills Ability to work independently in a home setting Reliable, compassionate, and professional demeanor Schedule Monday - Friday 4:00 PM - 10:00 PM Evening shifts (30 hours per week) Benefits 401(k) Dental insurance Health insurance Vision insurance Referral program Pay Competitive hourly rate based on experience Work Location Hawthorne, CA In-person home health setting
