Licensed Vocational Nurse (LVN) - Home Health Al Shifa Home Health Hemet, CA Job Details Per diem $30 - $45 an hour 1 day ago Benefits Flexible schedule Qualifications CPR Certification LVN BLS Certification Driver's License Basic life support Clinical documentation Care plans Nurse management Full Job Description Job Summary We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to provide high-quality care to patients in their homes. The LVN will work under the supervision of a Registered Nurse (RN) and collaborate with the interdisciplinary team to deliver personalized care that promotes patient independence, safety, and overall well-being. Responsibilities Provide direct patient care in the home setting in accordance with the plan of care (POC) Perform skilled nursing tasks including: Wound care and dressing changes Medication administration and management Vital signs monitoring and assessment Catheter care, ostomy care, and basic IV care (if applicable) Observe and report changes in patient condition to the RN and physician Assist with patient and caregiver education regarding: Disease process Medication regimen Safety and fall prevention Accurately document all care provided in a timely manner Maintain compliance with agency policies, state regulations, and infection control standards Coordinate care with RNs, therapists (PT/OT/ST), and Home Health Aides (HHA) Ensure a safe and clean environment for patients Qualifications Current California LVN license in good standing Current CPR certification (BLS) Minimum 1 year clinical experience (home health preferred, but not required) Experience with wound care, medication administration, and patient education preferred Valid driver's license, reliable transportation, and auto insurance Ability to travel to patient homes within assigned areas Skills & Competencies Strong clinical and assessment skills Excellent communication and interpersonal skills Ability to work independently and manage time efficiently Compassionate, patient-focused approach to care Knowledge of home health regulations and documentation Work Environment Field-based position providing care in patient homes Flexible schedule depending on patient needs and visit assignments
Compensation & Schedule Pay:
Competitive, based on experience (per visit or hourly)
