We are currently seeking a compassionate, organized, and team-oriented individual to join our Home Health team as a Full-Time Registered Nurse (RN) in Hercules, CA. Position Overview:

We are seeking an experienced, self-driven Full-Time Registered Nurse (RN) to join our dedicated Home Health team and provide exceptional care throughout the region. As a Registered Nurse (RN), you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care. Responsibilities:

Provide nursing care within the scope of practice, following the patient's individualized care plan, in coordination with the Interdisciplinary Team. Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing. Maintain accurate clinical records and submit documentation according to company policy using a company-provided device. Supervise Licensed Vocational Nurses (LVNs) when assigned. Comply with all company policies and procedures to ensure high standards of care.

Requirements:

Current Registered Nurse (RN) License to practice in California

• Required Experience in Home Health or Hospice care
• 1 Year Required Current CA Driver's License Able to travel within a 15
• 25 mile radius on a regular basis.
