Pay:
$2400-$2600/Wk plus .42 a mile for patient related travel Local and Travel Welcome! Irvine, CA 13 Week Contracts Schedule 1: Monday-Friday 8 AM-5 PM Guaranteed 40 hours per week Service Area:
Orange, Tustin, Anaheim, Irvine Must have at least 1 year of Hospice experience within the last 3 years as an RN. HCHB
charting experience is also required. Schedule 2: Monday-Friday 8 AM-5 PM Weekend rotation as needed. Service Area:
Seal Beach, Huntington Beach, Westminster, Garden Grove, Fountian Valley, Santa Ana, Costa Mesa, Newport Beach, Corona Del Mar. Must have at least 1 year of Hospice experience within the last 3 years as an RN. HCHB
charting experience is also required. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and medication reconciliation, ensuring accurate documentation within electronic health record (EHR) systems such as Epic or Cerner. Develop and implement personalized care plans that incorporate evidence-based practices for diverse populations, including geriatrics, pediatrics, and individuals with disabilities. Administer medications via IV infusion, injections, or other routes while adhering to aseptic techniques and safety protocols. Manage complex cases involving chronic conditions such as heart disease, respiratory issues, or post-surgical recovery, including wound care, suturing, and ostomy management. Provide education on disease management, medication administration, nutrition, and self-care strategies to patients and caregivers. Coordinate with multidisciplinary teams for discharge planning, case management, and referrals to specialists like cardiologists or infectious disease experts. Ensure compliance with HIPAA regulations and maintain meticulous medical documentation of patient interactions and interventions. Qualifications Valid RN license with current state registration; additional certifications such as Basic Life Support (BLS) are required. Proven experience in home health nursing or related settings such as hospital medicine, ICU, emergency room, or long-term care facilities. Familiarity with EMR/EHR systems like Epic or Cerner; experience with ICD coding (ICD-10/ICD-9) is preferred. Strong clinical skills including IV insertion, catheterization (urinary or central lines), phlebotomy, and sterile processing techniques. Knowledge of specialized areas such as hospice & palliative medicine, trauma medicine (Level I & II trauma centers), behavioral health, or geriatrics is highly desirable. Ability to perform comprehensive patient assessments in diverse environments; experience working with individuals with developmental disabilities or memory care needs is a plus. Excellent communication skills for triage calls and patient education; critical thinking and problem-solving abilities are essential for managing acute pain or infectious disease cases.
