Flexible Schedule Job Summary:

We are seeking a compassionate and reliable Licensed Vocational Nurse (LVN) to join our home health team on a per diem basis. The ideal candidate will provide skilled nursing care to patients in their homes, ensuring quality and comfort with each visit. Each visit is approximately 1 hour, with flexible scheduling options available.

Responsibilities:

Provide skilled nursing visits in patient homes Perform patient assessments and report significant findings to the RN case manager Administer medications and treatments as prescribed Educate patients and families on disease management and care plans Document all care accurately and timely using electronic charting Collaborate with the interdisciplinary team to ensure coordinated patient care

Requirements:

Current California LVN license Valid CPR certification At least 1 year of clinical experience preferred (home health a plus) Reliable transportation and valid driver's license Excellent communication and time management skills Comfortable working independently in the field
