Responsibilities:
Oncall hospice visits on weekends Provide accurate and timely documentation Assess and provide patient and family/caregiver education Perform other duties as assigned by the registered nurse Other visit as needed based on your availability including routine and PRN, visits, discharges, Deaths, CC, etc.
Qualifications:
Demonstrated compassion and commitment to patient centered care Active LVN license in the state of California Valid California driver's license and reliable transportation Minimum 1 year of hospice experience; strong understanding of hospice philosophy and end of life care Prior experience in nursing or related medical fields Experience as an on call LVN runner preferred
