Visits patients to assess and evaluate patient's status. Initiates the plan of care and makes revisions as needed. Administers medications and treatments as prescribed by the physician. Provides health care instructions to the patient & family. Completes accurate clinical notes in electronic EHR. Works in cooperation with the family/caregiver.

Position Qualifications:
