Qualifications:
Valid RN license in California 2+ years of recent experience in case management, preferably in acute care or hospital settings Demonstrated proficiency in patient advocacy, care coordination, and discharge planning Superior communication and interpersonal skills Ability to adapt to fast-paced, ever-changing healthcare environments
Key Responsibilities:
Collaborate with interdisciplinary teams to assess patient needs and design individualized care plans Coordinate transitions of care, including admission, ongoing care, and post-discharge support Conduct regular patient case reviews to ensure optimal quality of care and efficient resource utilization Advocate for patients and families, providing education and support regarding treatment and community resources Facilitate communication between healthcare staff, insurance providers, and external agencies
