Nurse Navigator Nuclear Care Partners - 3.7 Livermore, CA Job Details Full-time $47 - $64 an hour 1 day ago Benefits Dental insurance 401(k) Paid time off Employee assistance program Vision insurance Qualifications RN License Driver's License Full Job Description Nurse Navigator (RN) Livermore, CA 1:1 Patient Care! The Nurse Navigator is responsible for the day-to-day management, coordination and supervision of the related clinical activities within their assigned service area utilizing professional standards of care and agency policy and procedures. This is a non-exempt position. The Nurse Navigator supports and creates positive environments through a personal commitment to the Company's vision and values.
Compensation:
$47-$64 Pay based on years of verified working experience.

Option Shifts:
36-40 hours/week. Some weekends required
Responsibilities:
Expected to routinely complete face to face patient visits and staff visits in home setting for all patients on case load. Build face to face rapport with patient and patient families in implementing and managing care being rendered. Maintains communication with Regional Clinical Director, supervisor, case managers, RN Authorization Specialists, lead and team nurses, non-licensed staff, support staff and ancillary services. Coordinate with Clinical Director and implement staff development as required to maintain documentation compliance. Manages care implementation with attention to quality patient care and patient family satisfaction. Coordinates care authorization with Authorization Specialist regarding initial nursing assessments and new admissions. Implementation, evaluation and oversight of individual patient POC that corresponds with the physician's orders through assessment of patient's needs, condition, environment and consultation with other health team members. Performs patient assessment according to physician orders and completes nurse progress notes. Providing weekly updates to the patients in the Onboarding Process, giving them status of their applications, educating them on the EEOICPA program, development initial care expectations. Ability to create a Plan of Care in conjunction with the patient and their families. Ensures patient and patient-family are satisfied with level of care, plan of care and care team. Performs as scheduled and as needed Initial Nursing Assessments, Comprehensive Nursing Assessments, supervisory visits and new patient admissions. Provides home health aide initial certification training and ongoing education. Assists and supports HHA's through initial paperwork process. Provide clinical guidance and coaching for staff and patient families. Able to speak to the authorization process and the benefits of skilled nursing visits. Participates in home visits in service area per policies and procedures and as needed. Focuses on outreach and networking in territory per direction from management team and per company growth plan. When not in the field conducting company business should be available and in the office during office hours to enable accessibility of service for potential and current patients and families as well as staff and colleagues. Schedules visits with patients or fills shifts to ensure patient's needs are being met. Makes changes to routinely scheduled visits as needed. Provides assistance and support with patient advocacy. Assists with coordination of appointments, treatments and therapies. Performs appropriate preventative and rehabilitative nursing procedures.
Qualifications:
Bachelor of Science in Nursing preferred. Current RN licensure in the state of CA A qualified candidate must possess a strong sense of professionalism, effective interpersonal skills and be a true team player. Previous case management or leadership role(s) which would include Interviewing, hiring, on-boarding procedures, mentoring and leading are preferred. Must have a valid driver license and be in good standing
