Minimum Eligibility Requirements :
Current and valid California Licensed Vocational Nurse (LVN) license. Minimum of two (2) years of clinical experience in assisted living, skilled nursing, hospital, case management, discharge planning, or related healthcare setting. Strong knowledge of resident assessments, change-of-condition monitoring, and level-of-care determinations. Working knowledge of California Title 22 regulations, LIC forms, and RCFE licensing requirements. Ability to establish effective working relationships with residents, families, physicians, discharge planners, hospice agencies, and Community staff. Strong documentation, organizational, and time management skills. Proficiency with electronic health records and basic computer applications. Strong patient assessment skills with the ability to recognize urgent health concerns promptly. Familiarity with HIPAA regulations and medical records management. Excellent communication skills and a compassionate approach to patient service Valid California driver's license and ability to travel locally between sites as needed. Must successfully pass a background check and, where applicable, a health screening, in compliance with California regulations for residential care facilities.
Essential Functions:
Clinical Assessment and Admissions Conduct new pre-admission assessments for prospective residents at Vista Del Mar Senior Living. Perform reassessments for residents returning to Vista Del Mar Senior Living from hospitals or skilled nursing facilities, as assigned. Conduct all required assessments for Vista Del Mar Memory Care, including pre-admission, hospital return, and ongoing reassessments. Determine appropriateness of placement and level of care in accordance with Title 22 and RCFE licensing requirements. Complete all required pre-admission and assessment documentation accurately and in a timely manner, including Physician's Reports and supporting clinical records. Reassess newly admitted residents to confirm assessment accuracy and care needs. Identify trends related to admissions, discharges, and resident acuity and communicate findings to leadership. Resident Care Oversight Serve as the clinical lead for residents temporarily out of the Community, including hospitalizations, skilled nursing stays, hospice transitions, and other care settings. Review hospital and skilled nursing discharge paperwork upon resident return and ensure care plans, medications, and physician orders are updated accordingly. Provide oversight of hospice and home health services, including review of external provider documentation, forms, and orders. Ensure medication orders are received, implemented, and communicated appropriately, including collaboration with medication technicians. Monitor shower schedules, refusals, and care-related concerns; report findings to leadership and document appropriately in the electronic health record. Memory Care Clinical Support Serve as the primary assessment nurse for Vista Del Mar Memory Care. Provide clinical support to the Memory Care campus, including assessments, review of clinical documentation, and guidance related to changes in condition. Participate in Memory Care care-plan meetings as needed to support interdisciplinary collaboration and resident-centered care planning. Collaborate with Memory Care leadership and care teams to ensure continuity of care and regulatory compliance. Licensing and Regulatory Compliance Complete and submit required licensing reports, including LIC 624 and SOC 341 forms, within required timeframes if needed Ensure all licensing documentation is accurate, timely, and shared with appropriate Community leadership during assessment Maintain tracking systems for new admissions, reassessments, and ongoing resident evaluations. Ensure each resident has a current Physician's Report (LIC 602) on file, including annual updates, and follow up with physicians and hospice providers as needed. Maintain awareness of and compliance with applicable state, federal, and regulatory requirements, including HIPAA and confidentiality standards. Assisted Living Waiver and Special Programs Assist Community staff with reassessments for Assisted Living Waiver (ALW) residents every six (6) months or as required. Support Memory Care clinical initiatives, including tuberculosis testing and vaccination clinics for residents and staff. Participate in special clinical projects as assigned, including podiatry coordination and wellness initiatives. Communication and Collaboration Act as a liaison between hospitals, skilled nursing facilities, physicians, discharge planners, hospice agencies, residents, families, and Community staff. Educate residents and families regarding care needs, services, and transitions in a clear and compassionate manner. Attend assigned meetings and collaborate with nurse managers, care teams, and leadership to support resident outcomes. Provide timely follow-up to referral sources and internal teams regarding admission decisions and resident status. Administrative Responsibilities Track mileage and submit reimbursement reports as required. Submit referral, assessment, and activity reports as requested by leadership. Maintain professional appearance, demeanor, and conduct at all times. Perform other duties as assigned that support resident care, regulatory compliance, and Community operations. Physical and Sensory Requirements Ability to sit, stand, walk, and move throughout the Community and external care settings. Ability to lift and carry up to ten (10) pounds occasionally. Ability to see, hear, and communicate effectively with residents, families, staff, and healthcare professionals. Potential exposure to blood-borne pathogens consistent with healthcare environments.
