Qualifications 1. The center nurse is a registered nurse (RN) currently licensed by the California Board of Registered Nursing and has provided the center with a copy of his/her current license. 2. The center nurse will provide the center with a health examination, signed by a physician, and a TB clearance, which is not more than 12 months prior to employment or within 7 days of employment and shall provide the center a current certificate of CPR and First Aid training. B. Duties The duties of the RN include: 1. Assess each participant initially and subsequently to determine needs for nursing, personal care and assistance in activities of daily living such as bathing, grooming, toileting and eating, include these needs in the plan of care and supervise the provision of these services. 2. Develop the plan of care in collaboration with the participant's identified goals and/or concerns. 3. Completing at a minimum, quarterly and six-month reassessments, which includes evaluating and reviewing the progress made towards the participant's identified goal(s) collaboratively with participant and updating the plan of care as needed or desired by the participant and the collaborative process. a. Additional reassessments/updating of the IPC may also be completed in addition to the quarterly and six-month reassessments for the following reasons: when participant or his or her representative request, following inpatient hospital stay, after being away from the center following 30 days absence or a significant change in the participant's status, such as new diagnosis, new home environment, caregiving support, increased impairment/concerns in participant's mental/physical/emotional symptomology/status. 4. Attending and participating in all the multidisciplinary team (MDT) meetings. 5. Preparing an individual plan of care for each participant if nursing and/or personal care services are to be provided. 6. Administering medication and monitoring these medications to/for participants who require supervision, monitoring or reminders, assessing a participant's ability to self-administer medications and providing training and monitoring any participants who administer their own medications.• 7. Providing all nursing care specified in the participant plans of care. 8. Providing observation and monitoring of the participants' health status. 9. Providing health education and counseling to the participants and their families, when indicated in their plans of care. 10. Supervising the personal care services provided to the participants by the program aides. 11. Supervising any maintenance program(s) designated to be under the supervision of the RN. 12. Developing the plans of care for participants capable of benefiting from a continence-training program. 13. Assisting participants in obtaining medical services from providers other than the adult day health center staff. 14. Acting as a liaison to the participants' personal physicians and notifying them of any marked changes in the participants' conditions. 15. Providing emergency care, including arrangements for transportation when necessary. 16. May serve as a member of the Utilization Review Committee. 17. Providing dietary counseling and nutrition education to participants who are not on a therapeutic/special diet. 18. Maintaining the nursing component of the participant health records, which includes: a. Documentation of all nursing care provided, including the participant's reaction to treatment, on the day the service(s) is given. b. Countersigning all daily notes/flowchart entries documenting provision of a maintenance program(s), if the RN is designated as the supervisor. c. Documentation of all nutrition services (dietary counseling or health education) provided on the day the service is given. d. Documentation of the time and dosage of each drug administered to a participant. e. Quarterly progress notes for all participants receiving nursing and/or personal care services, and for all participants receiving maintenance program services if the RN is designated as the supervisor. f. Documentation of all initial assessments and, at least every six-months, reassessments for nursing and personal care services. g. Documentation of all individual plans of care developed for nursing and/or personal care services, and for maintenance program(s) if the RN is designated as the supervisor. h. Documentation of any marked changes in a participant's condition or unusual incidents observed by or reported to the RN. 19. Maintenance and supervision of all drug supplies and medications stored at the center and recording all these medications. 20. Destruction of any drugs which have expired or been left in the center 30 days after the participant's last visit. 21. Supervision of the licensed vocational nurse (LVN). 22. The RN is on duty full-time during the hours the center provides required services and reports to the Program Director. 23. Will be knowledgeable of
ADHC/CBAS
laws, regulations, CBAS certification standards, standards of participation and policies and procedures. This will be ensured through ongoing staff training and education.
