Licensed Vocational Nurse (LVN) Contract (Month-to-Month) |
Los Angeles, CA Pay Rate:
$35.00 - $37.00 per hour (DOE) Overview We are seeking compassionate and dedicated Licensed Vocational Nurses (LVNs) to provide high-quality care and support to adults with complex medical and mental health needs, including individuals experiencing homelessness and substance use disorders. These contract positions offer the opportunity to make a meaningful impact while working within a collaborative, trauma-informed, and harm-reduction-focused environment. Contract (Month-to-Month) Key Responsibilities Direct Nursing Care & Coordination Administer medications, injections, and other prescribed treatments. Monitor client health status and report significant changes to the appropriate providers. Coordinate primary care and specialty medical appointments. Maintain accurate medication logs and treatment records. Collaborate with pharmacies to ensure timely medication delivery. Provide health education and support to promote medication adherence and wellness. Collaborative, Integrated Care Work closely with an interdisciplinary team to provide comprehensive, client-centered care. Support clients with severe mental illness (SMI), co-occurring disorders, and complex medical needs. Assist with crisis intervention and coordinate emergency services when appropriate. Participate in team meetings, case conferences, and supervision sessions. Documentation & Compliance Complete timely and accurate documentation in the Electronic Health Record (EHR) system. Maintain compliance with HIPAA and all applicable healthcare regulations. Assist clients with navigating healthcare services and accessing available benefits. Ensure all documentation meets organizational, county, state, and funding-source requirements. Qualifications Education & Experience Experience in nursing, behavioral health, social services, or a related healthcare setting. One (1) to three (3) years of experience working with individuals experiencing severe mental illness, substance use disorders, homelessness, or other complex populations preferred. Skills & Abilities Strong clinical assessment and patient care skills. Ability to maintain professionalism and composure in challenging situations. Excellent communication and interpersonal skills. Experience using Electronic Health Records (EHRs) and healthcare technology. Understanding of trauma-informed care and harm-reduction principles preferred. Licenses & Certifications Current California Licensed Vocational Nurse (LVN) license (required). Current CPR certification (preferred). Valid California Driver's License with a clean driving record and insurability (required). Live Scan clearance (required).
