MDS Coordinator (Minimum Data Set Coordinator)

Los Angeles Position Summary:
The MDS Coordinator is responsible for managing and coordinating the completion of the Minimum Data Set (MDS) and related assessments for all residents in accordance with federal and state regulations. This role ensures the accuracy and timeliness of resident assessments, contributes to individualized care plans, and supports reimbursement and compliance processes under the Resident Assessment Instrument (RAI) system.
Key Responsibilities:
Assessment & Documentation Coordinate and oversee completion of all MDS assessments, care area assessments (CAAs), and related documentation. Ensure MDS assessments are accurate, comprehensive, and submitted within required timeframes. Monitor changes in resident condition and initiate significant change assessments as needed. Care Planning Collaborate with interdisciplinary team members to develop, update, and implement individualized care plans. Participate in resident care conferences to ensure care plans reflect residents' current needs and goals. Compliance & Quality Assurance Maintain current knowledge of CMS regulations and RAI guidelines. Conduct internal audits to ensure accuracy and compliance with documentation and coding standards. Assist with preparation for surveys and audits. Reimbursement Support Accurately capture data affecting case mix and reimbursement (PDPM or RUG-IV, as applicable). Work closely with the billing department to ensure MDS data supports reimbursement claims. Education & Training Provide education and support to nursing staff and interdisciplinary team members regarding MDS processes, documentation, and assessment requirements.
Qualifications:
Education:
Licensed Registered Nurse (RN) or Licensed Practical/Vocational Nurse (LPN/LVN) in good standing. Completion of an approved MDS training program (preferred).
Experience:
Minimum of 2 years of experience in long-term care or skilled nursing facility. Prior experience as an MDS Coordinator or in MDS/RAI processes strongly preferred.
Skills:
Strong knowledge of RAI process, PDPM, and CMS regulations. Excellent organizational and analytical skills. Proficiency with electronic health record (EHR) and MDS software systems. Effective communication and teamwork abilities.

Attention to detail and accuracy. Ability to work independently and manage multiple priorities. Commitment to resident-centered care and continuous quality improvement.
