is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to provide temporary coverage for an employee currently on leave. The LVN will provide client-centered nursing care within our Interim Housing Program, supporting residents experiencing homelessness, mental health challenges, substance use disorders, and chronic medical conditions. This temporary assignment will work under the direction of the Program Manager, with oversight from the Nursing Supervisor (when applicable), while ensuring compliance with all county regulations and ASF policies and procedures. Key Responsibilities Clinical Care & Monitoring Conduct daily wellness checks and assess changes in residents' physical or mental health. Assist with chronic disease management, including diabetes, hypertension, and asthma. Provide injections, wound care, dressing changes, vital signs, TB assessments, and first aid. Support medication monitoring, logging, counting, and compliance protocols. Complete accurate nursing documentation, including progress notes, MAR reviews, incident reports, and medication logs. Medication Support & Compliance Oversee medication storage, handling, and distribution in accordance with county regulations and ASF protocols. Review MARs daily and address discrepancies. Coordinate medication refills and deliveries with pharmacies and medical providers. Educate residents on medication use, side effects, and adherence. Report any medication concerns, refusals, errors, or health issues to leadership. Health Education & Advocacy Provide education on hygiene, wellness, chronic illness management, nutrition, and harm reduction. Assist residents with scheduling and attending medical appointments. Advocate for access to medical, dental, and behavioral health services. Crisis Response & Safety Respond to medical emergencies and behavioral health crises professionally and promptly. Collaborate with Program Managers, Case Managers, and supervisors to ensure resident and staff safety. Report incidents and unusual behaviors according to ASF policy. Collaboration & Documentation Work collaboratively with internal teams and external service providers. Participate in team meetings and case conferences. Maintain HIPAA compliance and confidentiality standards. Complete all required nursing documentation accurately and timely. Qualifications Valid California LVN license in good standing (required) Current CPR/BLS certification (required) Experience in behavioral health, homeless services, or medical-surgical settings preferred Strong communication, documentation, and organizational skills Ability to work independently in a fast-paced environment Ability to remain calm and professional during crisis situations Work Environment Interim Housing Program serving adults experiencing homelessness Requires standing, walking, and occasional lifting of up to 25 lbs. Exposure to medical emergencies and behavioral health crises
