Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history reviews using EMR (Electronic Medical Record) systems. Administer medications and injections accurately, including IV infusions, immunizations. Collaborate with multidisciplinary teams to develop and implement personalized care plans aligned with patient needs and clinical standards. Support patients with chronic conditions such as cardiology issues, infectious diseases, geriatrics, or behavioral health by delivering targeted interventions and education. Ensure compliance with HIPAA regulations and maintain meticulous medical documentation for legal and quality assurance purposes. Experience Proven experience of 5 years of working in Home Health Care. Demonstrated proficiency with EMR/EHR systems such as kinnser for documentation and order entry. Prior work involving complex patient populations such as geriatrics

$60.00 - $80.00 per hour
