Schedule:
5x8s Responsibilities Coordinate patient care across the continuum to ensure quality outcomes and efficient resource utilization. Facilitate discharge planning and transitions of care for patients and families. Conduct utilization reviews, case management assessments, and care coordination activities. Collaborate with physicians, social workers, nursing staff, and interdisciplinary healthcare teams. Monitor patient progress and identify barriers to discharge or treatment plans. Ensure compliance with regulatory requirements, payer guidelines, and organizational policies. Advocate for patient needs while promoting safe and effective care delivery. Maintain accurate and timely documentation of case management activities. Support patients and families through education and resource coordination. Requirements Active Registered Nurse (RN) License. Current BLS Certification. Recent Case Management experience required. Strong knowledge of discharge planning, utilization review, and care coordination. Excellent communication, organizational, and problem-solving skills. Ability to work collaboratively with multidisciplinary healthcare teams. Preferred Qualifications Acute care hospital case management experience. Experience with utilization management and payer regulations. Familiarity with electronic medical record (EMR) systems.
