The RN is responsible for providing comprehensive nursing assessments, developing and implementing the plan of care, managing complex medical needs, and delivering exceptional family-centered care while maintaining the highest standards of clinical excellence. The patient is a child with a traumatic brain injury (TBI) requiring continuous skilled nursing care, including management of a tracheostomy, mechanical ventilation, PICC line, MIC-KEY button G-tube, and total assistance with activities of daily living. The RN serves as the clinical leader during each shift, utilizing critical thinking skills to recognize subtle changes in condition and intervene appropriately. Essential Responsibilities Comprehensive Nursing Assessment Perform comprehensive head-to-toe nursing assessments at the beginning of each shift and as clinically indicated. Continuously evaluate respiratory, neurological, cardiovascular, gastrointestinal, and integumentary status. Identify changes in condition and initiate appropriate nursing interventions. Notify the physician, Director of Nursing, and family of significant clinical changes. Respiratory & Ventilator Management Manage and monitor mechanical ventilation. Assess ventilator effectiveness and patient synchrony. Perform tracheostomy care using sterile technique. Perform tracheal suctioning as needed. Change tracheostomy tubes as ordered and within competency. Administer supplemental oxygen per physician orders. Monitor oxygen saturation, respiratory effort, breath sounds, and airway patency. Respond immediately to ventilator alarms and respiratory emergencies. Maintain emergency airway equipment at the bedside. PICC Line Management Assess PICC line site each shift for signs of infection, infiltration, or mechanical complications. Perform sterile dressing changes per physician orders and agency policy. Flush PICC line according to established protocols. Administer IV medications, fluids, and therapies as ordered. Maintain central line patency using evidence-based infection prevention practices. Monitor for catheter-related complications and promptly report abnormalities. Gastrointestinal & Nutrition Manage enteral nutrition through the MIC-KEY button G-tube. Administer medications via G-tube. Assess feeding tolerance. Monitor hydration, bowel function, and nutritional status. Perform routine G-tube care and skin assessments. Recognize and respond to G-tube complications. Neurological Care Perform detailed neurological assessments. Monitor for seizure activity. Implement seizure precautions. Assess muscle tone, reflexes, cognition, and responsiveness. Recognize early neurological deterioration and initiate appropriate interventions. Medication Administration Administer medications by all appropriate routes, including oral, enteral, inhaled, intravenous, and intramuscular, in accordance with physician orders and RN scope of practice. Monitor therapeutic response and adverse effects. Verify medication compatibility for IV administration. Maintain accurate medication reconciliation and documentation. Skin & Mobility Complete comprehensive skin assessments. Develop and implement pressure injury prevention strategies. Reposition patient routinely. Perform passive range-of-motion exercises. Coordinate positioning for optimal respiratory function and comfort. Provide complete assistance with activities of daily living while preserving dignity and comfort. Equipment Management Safely operate and troubleshoot: Mechanical ventilator Suction machine Oxygen equipment Pulse oximeter Feeding pump Nebulizer equipment Enteral feeding equipment Ensure emergency equipment is available and functioning properly. Documentation Complete timely, accurate, and detailed nursing documentation. Document assessments, interventions, medications, treatments, ventilator management, PICC line care, patient response, and communication with providers and family. Maintain compliance with state regulations, agency policies, and accreditation standards. Family Education & Support Develop trusting relationships with the patient and family. Provide ongoing education regarding the patient's condition, equipment, medications, and plan of care. Reinforce physician recommendations and promote family confidence in care. Deliver care with compassion, professionalism, and respect. Clinical Leadership Coordinate care with physicians, respiratory therapists, therapists, pharmacists, durable medical equipment providers, and the interdisciplinary team. Provide guidance and clinical oversight to LVNs and caregivers when appropriate. Participate in care conferences and contribute to care planning. Advocate for patient safety and high-quality clinical outcomes. Required Qualifications Current California Registered Nurse (RN) license in good standing. Current BLS certification. Pediatric Advanced Life Support (PALS) preferred. Minimum of two years of pediatric critical care, PICU, NICU, home health, or private duty nursing experience preferred.

Excellent communication and documentation abilities. Reliable transportation and ability to work independently in a home setting. Preferred Qualifications Pediatric home health or concierge nursing experience. Experience caring for patients with traumatic brain injuries. Experience managing medically fragile pediatric patients requiring long-term mechanical ventilation. IV certification and demonstrated competency in central line management. Ability to stand, walk, bend, kneel, and lift as required throughout the shift. Ability to respond quickly and effectively during medical emergencies. Visual and auditory acuity sufficient to monitor ventilator alarms, infusion devices, patient assessments, and changes in clinical condition.
