Home Health Job Summary Seeking an experienced Wound Ostomy Nurse (RN) to serve as a clinical resource for patients and staff regarding wound care, ostomy management, continence disorders, skin breakdown, and fistula care. The nurse will provide direct patient care, education, and support for wound prevention and treatment initiatives. Requirements Active California RN License CWON (Certified Wound Ostomy Nurse) Certification Required BSN Required Minimum 4 years of clinical nursing experience 2+ years of wound and ostomy care experience preferred Valid driver's license, auto insurance, and reliable transportation Strong communication and interpersonal skills Responsibilities Assess and manage acute and chronic wounds, ostomies, and skin conditions Provide patient and family education before and after procedures Evaluate and monitor pressure injuries and hospital-acquired pressure injuries (HAPI) Collaborate with physicians, nurses, and healthcare teams on treatment plans Assist with wound care product evaluations and equipment recommendations Conduct staff education, in-services, and bedside teaching Promote wound prevention and infection control best practices Maintain knowledge of current wound and ostomy care standards Preferred Qualifications Home Health experience Wound and ostomy care experience Strong patient counseling and education skills Experience with clinical technology and wound care equipment Additional Information Local travel required No remote work available Monday-Friday schedule Home Health setting Immediate need for contract coverage
