As an independent contractor RN, you will be responsible for providing home health nursing care and assessment to patients in their homes. The ideal candidate should be flexible and adaptable and must be available to work on some weekend days.

Responsibilities:

Start of Care (SOC)

• Initial comprehensive assessment, OASIS completion, medication reconciliation, care plan development, and physician coordination. Recertification
• Evaluate continued eligibility for services and update the plan of care. Resumption of Care (ROC)
• Reassess patients returning home after hospitalization or rehab. Skilled Nursing Visits
• Ongoing clinical assessments, monitoring, and interventions. Discharge Visits
• Final evaluation, education, and transition planning. Supervisory Visits
• Oversight of LVNs or aides to ensure quality and compliance. Care Coordination
• Communicate with physicians, therapists, pharmacies, and families. Medication Management
• Reconcile medications, educate patients, and monitor effects. Wound Care
• Assess, treat, and monitor complex wounds. Patient & Caregiver Education
• Teach disease management, safety, and self-care. Documentation & Compliance
• Complete OASIS, visit notes, and regulatory documentation. Infusion Therapy
• Provide IV and central line care when qualified. On-Call / PRN Visits
• Address urgent clinical needs to avoid hospitalization.

Requirements:

Valid nursing license in California Minimum one year of experience as a home health RN. Experience with DeVero software Ability to assess, monitor, and manage patient care Excellent communication and interpersonal skills to establish rapport with patients, families, and interdisciplinary team members. Proficient in using electronic health records and other clinical documentation systems. Ability to work independently, manage time effectively, and prioritize tasks in a fast-paced environment. Reliable transportation and flexibility to travel to various patient locations as needed. Professional liability insurance coverage. Current CPR/BLS certification.
