Our community specializes in providing exceptional care and support to residents living with Alzheimer's disease and other forms of dementia. The ideal candidate is a strong clinical leader who is patient, compassionate, and committed to enhancing the quality of life for our residents. Excellent communication and interpersonal skills are essential, as this role works closely with residents, families, healthcare providers, and team members to ensure the highest standard of care. Responsibilities Conduct pre-admission assessments and develop resident needs and service plans; accurately enter and maintain resident information in PointClickCare. Direct, supervise, and continuously improve resident care systems and services. Ensure resident care and services meet Heritage Hills standards, regulatory requirements, and industry best practices. Coordinate and communicate resident care needs with residents' responsible parties, families, and designated representatives. Oversee medication management and administration processes for all residents. Collaborate with physicians, pharmacies, and other healthcare providers to obtain medical records, assess resident appropriateness for admission, and develop individualized care plans. Communicate changes in residents' conditions, medication concerns, and adverse reactions to physicians and other healthcare professionals in a timely manner. Team Member Leadership & Scheduling Develop and maintain staffing schedules that appropriately support resident care needs while remaining within budgetary guidelines. Recruit, supervise, coach, train, evaluate, and, when necessary, administer corrective action to team members to ensure high-quality resident care. Provide onboarding and ongoing training to team members regarding company policies, procedures, and applicable regulatory requirements. Facilitate dementia-specific education and training programs for new and existing team members to promote excellence in memory care services. Foster a positive, collaborative, and resident-centered culture among the care team. Position Requirements Current and active Licensed Vocational Nurse (LVN) license in the State of California required. Minimum of three (3) years of management and supervisory experience, including hiring, training, coaching, performance management, and employee development. Prior experience in Assisted Living and/or Memory Care is required. Current CPR and First Aid Certification required. Strong verbal and written communication skills, with the ability to effectively communicate in English. Demonstrated leadership skills with the ability to build positive relationships with residents, families, team members, and healthcare professionals. Strong business acumen, sound judgment, and the ability to exercise discretion and independent decision-making with minimal supervision. Excellent interpersonal, conflict resolution, analytical, problem-solving, and organizational skills. Ability to thrive in a fast-paced environment while effectively managing multiple priorities and deadlines. Proficient in maintaining accurate documentation and ensuring compliance with applicable regulations and company policies. Ability to push and pull up to 40 pounds and lift up to 35 pounds as required to support resident care and operational needs.
