In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will work closely with patients and their families, developing personalized care plans that address individual health needs. Your expertise in various medical fields will be essential in delivering exceptional care and support. Please note pay range provided is per case completed. Duties Conduct comprehensive assessments of patients' health status and needs. Develop, implement, and evaluate individualized care plans in collaboration with the healthcare team. Administer medications and treatments as prescribed, ensuring adherence to safety protocols. Monitor patients for changes in condition and respond appropriately to any emergencies. Educate patients and families on disease management, medication administration, and preventive health measures. Utilize OASIS assessment and documentation via DSL (Data Soft Logic) interface Perform periodic re-evaluation/ re-certification of the patient as necessary and makes adjustments in the patient's treatment program. Collaborate with multidisciplinary teams to coordinate care for patients with complex medical conditions, including infectious diseases and internal medicine issues. Uphold infection control standards to ensure the safety of both patients and healthcare providers. Experience Valid Registered Nurse (RN) license in the state of practice. Strong knowledge of anatomy and physiology, coding practices, and infection control measures. Experience in home health care or similar settings is preferred. Familiarity with systems used in patient management, particularly DSL Ability to work independently while demonstrating strong critical thinking skills.
