Travel Nursing Contract Key Responsibilities:

Coordinate and manage patient care plans to ensure quality outcomes Perform discharge planning , care coordination, and utilization review Collaborate with interdisciplinary teams ( IDT collaboration ) for patient-centered care Conduct comprehensive patient assessments and care evaluations Ensure compliance with healthcare standards and payer requirements Advocate for patients through proper resource utilization and continuity of care Improve patient satisfaction and cost-effective care management

Requirements:

Active California RN License (Required) BLS Certification through American Heart Association (AHA) Minimum 5 years of recent RN Case Management experience Experience in LTAC, STACH, Acute Care, or Hospital Case Management settings Strong knowledge of MCG Guidelines, discharge planning, utilization review, and care coordination Excellent communication and interdisciplinary collaboration skills

Preferred Experience:
