Job Description:
POSITION SUMMARY
The Case Manager is a Registered Nurse (RN) who uses a collaborative process which assesses plans, implements, coordinates, monitors and evaluates options and services to meet an individual's health needs through communication and available resources to promote quality cost-effective outcomes. The Case Manager applies each of these six essential activities within each of the following five core components: Coordination and Service Delivery, Physical and Psychological Factors, Benefit Systems and Cost Benefit Analysis, Case Management Concepts, and Community Resources. These activities and core components are applied to promote wellness on the continuum of care.
POSITION QUALIFICATIONS
Education/Licensure/Certification:
Current State of California Registered Nursing License required Case Management National Certification preferred, or achieved within two years of employment. Current Basic Life Support (BLS) certification required.
Knowledge:
Minimum of two years of nursing experience in a variety of settings preferred. Experience where leadership, integrity, initiative and effective communication skills have been demonstrated. Knowledge of community resources and healthcare financial system required. Knowledge of discharge planning and utilization review preferred.
Skills:
Excellent verbal and written communication skills. Must exhibit openness to change and a willingness to try new approaches. Experience with team building, consensus building and group facilitation. Ability to prioritize work assignments appropriately. Must have the physical and mental health adequate to meet the demands of the job.
