After-Hours RN Mission Hospice and Home Care - 4.2 Pleasanton, CA Job Details Per diem $52.90 - $66.20 an hour 23 hours ago Qualifications RN License Full Job Description Mission Hospice & Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end-of-life support and education. Founded in 1979, we are a local, independent nonprofit that has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life-threatening diagnosis through death and bereavement. This position offers competitive pay, and a supportive work environment. We are seeking a per diem After-Hours Nurse to provide skilled nursing and hospice care to maximize the comfort and health of patients.
Schedule:
Saturday & Sunday; 4:30pm - 12:30am
Office Location:
San Mateo, CA Coverage Area:
San Mateo, Alameda & Contra Costa Counties Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel. Essential Duties & Responsibilities include (but are not limited to): Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations. Under the guidance of the After-Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care. Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/ family. Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues.
Education/Experience :
One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing. Experience in acute care/ home health and/or hospice setting preferred .
Certificates, Licenses, Registrations:
Current license as a registered professional nurse in the State of California. Public Health Nursing certification highly desirable. HPNA certification preferred.
Pay Range:
