Field Nurse The Wound Well Company Pomona, CA Job Details Part-time | Full-time $33.74 - $40.64 an hour 1 hour ago Benefits Health insurance Qualifications Nursing Teamwork Skilled nursing facility experience RN License Time management Full Job Description Travel Wound Care Nurse (LVN/RN)Position Summary As a Travel Wound Care Nurse (LVN or RN) , you will provide direct patient care across assigned facilities and home-based settings while supporting advanced wound care initiatives and clinical coordination. This role requires flexibility, strong clinical judgment, and collaboration with interdisciplinary care teams to deliver high-quality, patient-centered care. This position plays a critical role in both hands-on clinical care and leadership support within specialty treatment programs, including wound management, Aerobella therapy, and skin integrity initiatives. Registered Nurses (RNs) will serve in an expanded clinical leadership capacity, providing advanced assessment, care planning, and mentorship to clinical staff. Essential ResponsibilitiesClinical Care & Patient Management Perform comprehensive patient assessments and contribute to individualized care planning. Provide direct wound care treatment and support evidence-based wound management protocols. Monitor wound healing progress, identify changes in patient condition, and communicate recommendations to providers. Deliver patient and caregiver education regarding wound care, treatment plans, and prevention strategies. Ensure adherence to infection prevention and safety standards. Aerobella Therapy Support Support Aerobella therapy patients through assessment, treatment oversight, monitoring, and patient education. Assist with implementation and follow-up of prescribed specialty therapies. Monitor treatment effectiveness and report outcomes to providers and care teams. Care Coordination Coordinate patient visits across facilities, homes, and other care settings. Collaborate with physicians, nurse practitioners, facility staff, caregivers, and internal operations teams to ensure continuity of care. Facilitate communication between all members of the healthcare team to optimize treatment adherence and patient outcomes. Skin Integrity & Prevention Programs Participate in Skin Sweep Team initiatives focused on prevention, early identification, and intervention for skin breakdown. Assist with facility-wide skin integrity assessments and preventive care programs. Support quality improvement efforts related to wound prevention and management. Documentation & Compliance Complete accurate, timely, and thorough clinical documentation in accordance with company policies, regulatory requirements, and professional standards. Maintain compliance with HIPAA, state nursing regulations, and organizational protocols. Document treatments, assessments, patient responses, and care coordination activities. Travel & Field Operations Travel regularly between assigned patient locations, facilities, and service areas. Maintain flexibility to support changing patient schedules and organizational needs. Represent Wound Well professionally in all clinical and facility interactions. Additional Responsibilities for Registered Nurses (RN) Serve as a clinical resource and mentor for LVNs and other clinical staff. Support clinical leadership initiatives and assist with training and competency development. Participate in complex patient assessments and treatment planning. Provide guidance on clinical decision-making and escalation of care concerns. QualificationsLicensed Vocational Nurse (LVN) Active California Licensed Vocational Nurse (LVN) license in good standing. Minimum of two years of clinical nursing experience preferred. Experience in wound care, home health, skilled nursing, specialty treatments, or mobile healthcare preferred. Strong assessment, organizational, and communication skills. Ability to work independently while collaborating effectively with interdisciplinary teams. Registered Nurse (RN) Active California Registered Nurse (RN) license in good standing. Minimum of two years of clinical nursing experience preferred. Experience in wound care, specialty treatments, home health, skilled nursing, or mobile healthcare preferred. Advanced assessment, critical-thinking, and clinical decision-making skills. Experience providing clinical guidance or mentoring preferred. Preferred for Both LVN and RN Wound Care Certification (WCC, WOCN, or equivalent) preferred but not required. Experience with Aerobella therapy or willingness to complete company-sponsored training. Strong organizational and time-management skills. Excellent interpersonal and communication abilities. Reliable transportation, valid driver's license, and willingness to travel regularly. Physical Requirements Ability to stand, walk, bend, kneel, and lift up to 25 pounds as needed. Ability to travel between multiple patient care locations throughout the workday. Ability to perform clinical procedures and patient assessments in various care settings. Why Join Wound Well?
