The ideal candidate will have extensive experience across various clinical settings, including acute care, critical care, emergency medicine, geriatrics, and outpatient services. This role involves providing comprehensive patient care, performing assessments, administering treatments, and collaborating with multidisciplinary teams to ensure optimal health outcomes. Proficiency with charting using electronic health record systems such as OASIS is essential. The Registered Nurse will also demonstrate expertise in specialized procedures including IV infusion and infection control. Candidates with experience in home health care, hospice & palliative medicine, trauma medicine, or working with patients with disabilities are highly preferred. This position offers an opportunity to make a meaningful difference in patient lives within a dynamic healthcare environment. Responsibilities Conduct thorough patient assessments including vital signs, physical examinations, and medical history documentation using EMR/EHR systems. Administer medications and injections accurately while adhering to aseptic techniques and HIPAA regulations. Perform specialized procedures such as IV insertion, catheter care, tube feeding management, and phlebotomy. Coordinate discharge planning and case management for post-acute care patients across settings such as nursing homes, assisted living facilities, or home care environments. Maintain compliance with infection control standards. Educate patients and families on health coaching, medication administration, nutrition, and chronic disease management including diabetes or infectious disease care. Document all patient interactions thoroughly in medical records. Participate in quality improvement initiatives aligned with NCQA standards and hospital policies for patient safety. Experience Proven clinical experience in diverse healthcare environments such as hospitals (including trauma centers), outpatient clinics, urgent care centers, or long-term care facilities. Demonstrated proficiency with EMR/EHR charting systems like OASIS. Experience performing advanced nursing procedures including IV infusion, tube feeding management, ostomy care, or wound care.
