General Job Description:
The Registered Nurse Care Manager (RNCM) works under the general direction of the Director of Patient care and Services (DPCS). The RNCM serves as a member of the Interdisciplinary Team (IDT) and coordinates patient care consistent with the Plan of Care (POC) and is responsible for provision of the service consistent with the mission, vision, and goals of Prestigious Hospice, quality outcomes and regulatory standards.
Duties and Responsibilities:
Assumes primary responsibility for assigned patients and performs all phases of the nursing process. Communicates with attending physicians, Prestigious staff members, and other agencies as necessary to coordinate optimal care and use of resources for patients/caregivers. Educate patients/caregivers regarding hospice philosophy, disease progression, physical components of care, stages of dying and other aspects of the POC as necessary. Documents all patient related information according to Prestigious Hospice's policies and procedures. Seeks input from IDT members regarding the patient POC to obtain additional knowledge and support. Reviews and updates the POC at least 14 days per regulatory standards. Regularly attends and participates in meetings to ensure that the POC reflects current patient care needs. Prepares in advance for IDT meetings. Assesses need for Certified Home Health Aide (CHHA) services. Conducts CHHA supervisory visits per regulatory standards. Assess the need for and make revisions to the CHHA care plan. Informs the DPCS of unusual or potentially problematic patients/caregiver issues as identified. Assists other RNCM's with their patient assignments as needs are identified. Provides on-call coverage as scheduled. Maintains annual clinical competencies. Identifies patients who may potentially revoke or need to be discharged. Models prompt and regular attendance. Exhibits a cost effective set of time, mileage, supplies and other agency resources. Demonstrate familiarity with and adheres to the policies and procedure of Prestigious Hospice, state, federal and accrediting agency's requirements. Provides notification of absence per Prestigious Hospice policy. Promotes and maintains respect for rights, privacy and confidentiality of others. Respects patients and staff confidentially at all times. Attends staff meetings, mandatory in-service, and other meetings as appropriate or upon requests. Participates in committees, task forces and Prestigious Hospice events as requested. Assumes responsibility for own personal and professional growth including state licensing/certification requirements. Understands and exemplifies hospice philosophy, values, beliefs and team concepts in all job-related roles. Abides by dress code. Wears identification/names badge during working hours according to Prestigious Hospice's policy. Other duties assigned.
