Job Description

The Licensed Vocation Nurse (LVN) Case Manager will perform professional nursing duties including assessments, treatment and follow through resolution of all health issues on-site for the 4 residents in each of our EBH residential programs in Escondido and Ramona, CA.
The LVN Case Manager:
Provides overall medical support to individuals who receive services to include scheduling medical appointments and follow-up as needed. Works closely with Program Director and Administrator to ensure health and safety of individuals receiving services. Provides on-site medical support to individuals. Completes documentation using Avatar and Chartmeds Maintain frequent contact with family members and external stakeholders as needed. Represents Merakey at all external meetings, conferences, etc. as assigned. Is knowledgeable and adheres to program regulations, assisting in preparation for annual licensure as applicable Completes other duties as assigned Earn $40-$45 per hour based on experience. The ideal candidate will possess the following qualifications:
LPN/LVN CA
license Valid driver's license 1 year experience working with individuals with developmental disabilities and/or mental health issues preferred Experience working in a residential environment preferred. Case management experience preferred You will work Monday-Friday day shift (typically 8:00 a.m.-4:30 p.m.) with some flexibility needed on occasion to accommodate early morning or evening medication test-outs, weekends and holidays off!
