Evaluates patients referred to the Organization; identifies insurance coverage, negotiates payment or per diem rates; plans and coordinates referrals for home care services. This energetic and compassionate position offers the opportunity to utilize your extensive clinical expertise to improve health outcomes and enhance quality of life. You will work closely with patients, families, and multidisciplinary teams to develop personalized care plans, facilitate smooth transitions across care settings, and ensure adherence to best practices in healthcare delivery.
Duties:
Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine individual needs. Collaborate with healthcare providers to optimize patient care strategies. Develop and implement discharge planning processes that incorporate diagnostic evaluations, utilization management, and patient education. Monitor patient progress through medical records review and diagnostic testing results; adjust care plans accordingly to meet evolving needs. Facilitate intake procedures for new patients while maintaining strict compliance with HIPAA regulations and medical terminology standards. Engage in case management activities that include hospital experience insights, outpatient clinic coordination, and post-acute follow-up to promote recovery and prevent readmissions.
Qualifications:
Registered nurse with current licensure to practice professional nursing in the state. Graduate of an accredited school of nursing. Minimum of two (2) years experience, at least one (1) of which is in the area of public health or home care nursing. Knowledge of physiology principles combined with expertise in patient assessment techniques such as vital signs monitoring. Experience working within managed care environments and utilizing utilization management tools effectively. Strong understanding of medical records documentation, diagnostic evaluation procedures, discharge planning processes, and outpatient clinic workflows. Excellent verbal and written communication skills with the ability to collaborate across multidisciplinary teams and primary care providers. Demonstrates an ability to work with other health care clinicians, development of home plan of care, knowledge of third party reimbursement, and ability to negotiate payment rates.
Pay:
$80,000.00 - $90,000.00 per year
