This position is ideal for an LVN with a strong clinical background, exceptional organizational skills, and experience with regulatory compliance and medical record management. Responsibilities Conduct routine quality assurance and medical record audits across multiple facilities. Review admissions, weekly, monthly, quarterly, and annual documentation for accuracy and compliance. Identify missing or incomplete documentation and follow up with facility staff to ensure timely correction. Audit physician orders, care plans, treatment records, and nursing documentation. Assist with QAPI initiatives, survey preparedness, and performance improvement projects. Monitor documentation compliance and communicate findings to leadership. Support facilities in maintaining organized and compliant medical records. Travel between assigned facilities as needed to complete audits and provide support. Perform additional QA and medical records duties as assigned. Qualifications Current and unrestricted California LVN license. Previous QA and/or Medical Records experience required. Experience using PointClickCare (PCC) is required. Strong understanding of regulatory documentation standards and compliance. Excellent organizational, communication, and follow-up skills. Ability to work independently and manage multiple priorities. Must have reliable transportation and be willing to travel between facilities throughout the assigned service area.
