Position Overview:

Navitas Healthcare, LLC is seeking a Registered Nurse

• Case Manager for an exciting Travel or Local job in Eureka, CA.

Key Responsibilities Perform case management duties for acute care patients, including assessment, planning, and coordination of care. Develop individualized care plans in collaboration with physicians, nurses, and interdisciplinary teams. Facilitate patient transitions, including discharge planning, referrals, and follow-up care coordination. Communicate with patients, families, and care providers regarding treatment plans and care needs. Ensure compliance with facility policies, procedures, and regulatory standards. Participate in unit assignments and float between units as needed to support patient care. Maintain accurate and timely documentation in the Epic electronic medical record system. Assist patients with resources and services, including housing support for travelers if required. Required Qualifications Active Registered Nurse (RN) license in California. Minimum of 2 years of acute care nursing experience (first-time case managers may be considered with clinical approval). Experience in case management or care coordination preferred. Strong communication, organizational, and critical thinking skills. Ability to work every other weekend as scheduled. Proficiency with electronic medical records, preferably Epic.
