Hospice/Home Health RN - Per Diem Solid Care Home Health Riverside, CA Job Details Per diem $80 - $225 an hour 2 hours ago Qualifications CPR Certification Skilled nursing facility experience RN License BLS Certification Driver's License End-of-life care Full Job Description NeoCare Palliative & Hospice (Pure Bliss Hospice Care, Inc.) is a hospice in Woodland Hills, CA that specializes in end of life comfort, for ALL ages. Our team works close with the patient, families and caregivers of the patient along with the physician to achieve seamless physical, psychosocial and spiritual care. We're seeking a Registered Nurse to join our hospice and home health team. Primarily responsible for providing professional nursing care with input from the IDT (interdisciplinary team), patient and family, assesses, develops, implements and evaluates the plan of care to meet the needs of individuals and families within their home environments. Specializing care to young, middle and older adults. This position must cover the surroundings cities like: Corona, Colton, Beaumont, Perris and others.
Responsibilities:
Handle a caseload of assigned patients. Conduct home visits for patients (Travel required for patient visits). Performs admission assessment and Start of Care's Develops written plan of care in coordination with an interdisciplinary Team (IDT). Assess patient needs and provide appropriate nursing services. Oversees implementation of plan, evaluates and changes plan as necessary to meet patient/family needs. Communicates all changes to IDT and primary MD. Maintains timely patient documentation (Charting) Complete reports and patient assessments Implements and maintains established hospice policies and procedures as well as Standards of Care and regulatory requirements.
Qualifications:
Must have current California Registered Nurse License 1 year of recent nursing experience in hospice, palliative care, home health, acute care &/or skilled nursing (REQUIRED) Current CPR/BLS Current CA driver's license with proof of insurance. Must be located within the same area
Pay:
$80.00 - $225.00 per hour
