We are seeking experienced Registered Nurses (RNs) to join our float/on-call team supporting multiple Congregate Living Health Facilities (CLHFs). Our facilities provide care up to a subacute level, including residents with tracheostomies and ventilator support in smaller, more controlled care environments. This role functions similar to registry nursing, with PRN shifts posted as needed across multiple facilities. Nurses must be comfortable adapting to different teams and working independently while delivering high-quality resident care.

This is an on-call/PRN float position. Hours are not guaranteed.

Pay:

$65.00 per hour Shifts available

• 12-hour shifts 6:00 AM
• 6:30 PM 6:00 PM
• 6:30 AM Facilities you may be scheduled at: Arrowhead Congregate Living Health Facility LLC (Colton) Oak Grove Manor LLC (San Bernardino) Sunny Sands Manor (Perris) Typical patient ratio: Each nurse is typically responsible for 6-9 residents, allowing for more focused care than many larger skilled nursing facilities.

Responsibilities:

• Provide direct nursing care based on residents' individualized care plans
• Administer medications and treatments as ordered
• Perform tracheostomy care, suctioning, and ventilator monitoring
• Monitor residents' conditions and report changes to the appropriate provider
• Document all care accurately in PointClickCare (PCC)
• Collaborate with facility staff to ensure resident safety and continuity of care
• Respond promptly to changes in resident condition

Requirements:

• Active California RN license in good standing
• Minimum one (1) year SNF or subacute experience required
• Trach and ventilator experience

REQUIRED

• New graduates will not be considered
• PointClickCare (PCC) experience

REQUIRED

• Current BLS certification

REQUIRED

• Strong clinical skills and ability to work independently
• Must be willing to travel between facilities as needed

Pay:
