Per Diem (but have availability Mon-Fri) Job Overview We are seeking an experienced and compassionate Registered Nurse (RN) to join our hospice team as a Per Diem Case Manager serving Rohnert Park and surrounding communities. This role is ideal for a nurse who values patient-centered care, autonomy, and meaningful connections with patients and families during end-of-life care. In this vital role, you will deliver holistic nursing care, coordinate interdisciplinary services, and educate families to ensure a peaceful and meaningful experience for patients and their loved ones. •RATES ARE PER VISIT, NOT PER HOUR. RATES ARE DETERMINED BY EXPERIENCE
• Responsibilities Conduct comprehensive patient assessments including vital signs, symptom management, and physiological evaluations to develop individualized care plans. Administer medications and treatments such as IV infusions, pain management protocols, and medication administration aligned with hospice standards. Collaborate with interdisciplinary teams to coordinate services like palliative therapies, psychological support, and spiritual care that enhance patient comfort. Educate patients and families on disease progression, symptom control techniques including ostomy care or tube feeding, and advance directives to facilitate informed decision-making. Document all patient interactions accurately using electronic health record (EHR) systems such as Epic or Cerner while maintaining compliance with HIPAA regulations. Provide emotional support and counseling to patients experiencing acute pain or distress, ensuring compassionate end-of-life care. Participate in ongoing training on topics like hospice & palliative medicine, infectious disease care, and trauma medicine to stay current with best practices. Qualifications Valid nursing license with current registration in the state of California Proven experience in hospice & palliative medicine or related fields such as critical care, hospital medicine, or home care. Familiarity with EMR/EHR systems including Wellsky/Kinnser for accurate medical documentation. Ability to handle complex cases involving acute pain management, dementia or Alzheimer's care, behavioral health issues, and working with individuals with disabilities or developmental delays. Excellent communication skills for patient assessment, family education, and interdisciplinary collaboration.
