Per Diem/On Call(Marin+Petaluma+Sonoma+Santa Rosa+Rohnert Park) Aura Hospice Rohnert Park, CA Job Details Per diem $50 - $60 an hour 1 day ago Qualifications Vital signs Medication administration LVN Hospice experience HIPAA End-of-life care Full Job Description Job Summary Join our dedicated hospice team as a Per Diem On-Call Licensed Vocational Nurse (LVN) specializing in hospice care within Sonoma/Santa Rosa/Marin/Petaluma and surrounding areas. In this vital role, you will provide compassionate, patient-centered nursing care to individuals at the end of life, ensuring comfort, dignity, and support for both patients and their families. This flexible on-call position offers the opportunity to make a meaningful difference while working in a dynamic healthcare environment. You will respond promptly to patient needs, assess symptoms, and coordinate care plans, all while maintaining compliance with healthcare regulations and standards.
On Call hours would be:
Mon-Fri:
5pm-8am
Sun-Sat:
all day •must be willing to travel all of Petaluma, Sonoma, Santa Rosa, Rohnert Park, Marin cities
Compensation:
Stipend + per visit rate + mileage Responsibilities Respond to on-call requests promptly, providing expert triage and assessment of patient needs in a hospice setting. Conduct comprehensive patient assessments including vital signs, symptom management, and physical examinations. Administer medications, including injections and IV infusions, following strict protocols and documentation standards. Collaborate with interdisciplinary teams to develop and update individualized care plans that prioritize comfort and quality of life. Educate patients and families on disease progression, medication management, and comfort measures such as tube feeding or catheter care. Maintain accurate medical documentation within Electronic Health Record (EHR) systems like Epic or Cerner, ensuring HIPAA compliance. Provide guidance on emergency situations such as acute pain management or airway management, escalating as needed to higher levels of care like ICU or trauma centers. Experience Valid LVN license with current state licensure required. Prior experience in hospice care or palliative nursing strongly preferred. Critical care experience including ICU or emergency medicine is highly desirable. Familiarity with EMR/EHR systems such as Wellsky/Kinnser Experience working with diverse populations including individuals with developmental disabilities, dementia, Alzheimer's disease, or memory care needs. Background in home & community care settings including assisted living, nursing homes, or hospital environments such as Level I Trauma Centers or Level II Trauma Centers is advantageous. Skills in patient assessment, medication administration (including injections), IV insertion, venipuncture, catheterization, and sterile technique. Ability to handle complex cases involving acute pain management, infectious disease care, and end-of-life support with compassion and professionalism.
