Home Health RN Primetime Home Health Agency Sacramento, CA Job Details Part-time | Per diem $100 - $120 an hour 15 hours ago Benefits Flexible schedule Qualifications RN License Healthcare infection prevention and control expertise Home health Full Job Description Job Overview We are seeking a dedicated and compassionate Home Health Registered Nurse (RN) with minimum 1 year of RN experience to provide exceptional patient care in the comfort of patients' homes. This vital role involves delivering comprehensive nursing services, conducting detailed assessments, and developing personalized care plans to promote health, recovery, and well-being. The ideal candidate will thrive in a dynamic environment, possess strong clinical skills, and demonstrate a passion for improving lives through expert home-based care. Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and detailed health histories. Administer medications, including injections and IV infusions, following strict aseptic techniques and safety protocols. Manage complex wound care procedures such as suturing and ostomy care while ensuring optimal healing outcomes. Coordinate with interdisciplinary teams to develop and implement individualized care plans aligned with patient needs. Educate patients and their families on disease management, medication administration, and lifestyle modifications to foster independence. Monitor patients' progress through regular follow-ups, adjusting care plans as necessary based on clinical evaluations. Document all patient interactions accurately within electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations. Experience Minimum 1 year of proven experience as a Registered Nurse in home health or related settings such as medical-surgical units, ICU, or emergency medicine. Demonstrated proficiency with EMR/EHR systems including Kinnser/WellSky for accurate documentation. Strong clinical background in areas such as critical care, cardiac catheterization, dialysis, hospice & palliative medicine, geriatrics, and trauma medicine. Experience working with diverse patient populations including pediatrics, geriatrics, individuals with developmental disabilities or dementia care needs. Knowledge of infection control practices and sterile processing procedures essential for maintaining patient safety. Familiarity with advanced nursing skills such as airway management, tube feeding management, laboratory specimen collection/processing, and cardiopulmonary assessments. Ability to perform complex procedures including phlebotomy, venipuncture, catheterization (urinary or vascular), and basic life support (BLS). Prior experience in various healthcare environments such as hospital medicine units, outpatient clinics, assisted living facilities or nursing homes is highly valued.
