  • $3000 (DOE) We are seeking a dedicated Registered Nurse (RN)
  • Case Manager to support an Adult Stem Cell Transplant Program in an outpatient oncology clinic setting.
This role involves coordinating patient care, managing referrals, and collaborating with multidisciplinary teams to provide seamless care for oncology patients.
Job Duties:
Coordinate patient care and treatment plans throughout the continuum of care Manage new patient referrals and authorization processes Collaborate with physicians and interdisciplinary teams to coordinate appointments and services Monitor and respond to patient communication through electronic health records and patient messaging systems Provide patient and family education related to treatment plans and care processes Support symptom management and ongoing patient follow-up Review and communicate abnormal lab values and patient status updates Coordinate care for patients with leukemia, lymphoma, multiple myeloma, and stem cell transplant needs Maintain accurate patient documentation in EMR systems Ensure compliance with infection control, isolation, and neutropenic precaution protocols
Requirements:
✔ Active RN License ✔ BLS Certification required ✔ Minimum 1 year of Oncology RN experience required ✔ Outpatient Oncology experience required ✔ Bone Marrow Transplant (BMT) experience preferred ✔ Epic experience preferred
Preferred Certifications:
OCN BMTCN ONS/ONCC
Chemotherapy Certification Ideal Candidate:
Strong communication and care coordination skills Comfortable working in a fast-paced oncology clinic environment Excellent organization and multitasking abilities Team-oriented with a patient-centered approach
Pay:
$2,600.00
  • $3,000.
