The primary responsibilities for this position consist of providing total, comprehensive, continuous and patient centered care which includes assessing, documenting, planning, implementing, teaching, developing, and evaluating a comprehensive nursing plan of care; delivering direct patient care; ensuring patient satisfaction; training and delegating duties; and adhering to university policies and procedures as well as federal, state and local regulations. Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care. Provides direct patient care to a diversified patient population. Applies knowledge, experience, and judgment to determine the importance of a situation, set priorities, and use abstract thought to evaluate clinical situations. Participates in patient and staff education. Demonstrates leadership skills. Demonstrates accountability for own practices as defined by the Nurse Practice Act.

  • Research Department Overview The Pulmonary Specialty Clinic provides patient care to 1200 patients per month.
There are several subspecialties seen here at this location. These include asthma, COPD, interstitial lung disease, pulmonary arterial hypertension, sarcoidosis, bronchiectasis and adult cystic fibrosis. The pulmonary clinic team is multidisciplinary with medical assistants, licensed vocational nurses, registered nurses, respiratory therapists, patient service representative 2 and Patient Service Representative 3 staff. There are 24 faculty, 15 fellow physicians and 2 nurse practitioners. Incoming call volume to our call center is approximately 2500 calls per month. Department Specific Job Scope The transplant coordinators in the Advanced Lung Disease (ALD) subspecialty at UC Davis Health provide care to adults with a variety of ambulatory care needs. The transplant coordinator II is a nursing clinical expert in advanced lung disease, and lung transplantation. This position has responsibility for the clinical case management of a large population of advanced lung disease patients, namely pulmonary hypertension. Additionally, the position coordinates the lung transplant evaluation of a subset of these patients and case manages them through the pre-transplant phase as well as the long-term post-transplant phase. The responsibilities involve coordination of the interdisciplinary medical care delivery system under the direction of the Medical Director. The transplant coordinator II functions as the liaison between the medical team, the patient, the family, referring physicians, multiple transplant programs, and insurance companies. This position provides intense patient and family education. This position assesses appropriate medical intervention for effective resource management and quality assurance. This position assumes shared responsibility for on-call patient triage activities, outside business hours for the advanced lung disease and lung transplant populations. This position applies advanced knowledge and patient assessment skills in lung disease and lung transplant to perform this function.
