In this vital role, you will deliver exceptional patient care across diverse settings, including acute care hospitals, outpatient clinics, long-term care facilities, and community health environments. Your expertise will help ensure patients receive safe, effective treatment while supporting their recovery and well-being. This position offers an exciting opportunity to utilize your clinical skills, work with multidisciplinary teams, and make a meaningful difference in patients' lives every day. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Point Click Care. Administer medications and injections accurately, following aseptic techniques and safety protocols. Manage complex care needs such as dialysis, ostomy care, and wound . Coordinate patient care plans through case management activities, discharge planning, and collaboration with physicians, therapists, and other healthcare professionals. Conduct diagnostic evaluations including specimen collection/processing and laboratory testing to support clinical decision-making. Provide education on health coaching topics such as geriatrics, memory care, behavioral health, pain management, and family planning to patients and their families. Ensure compliance with HIPAA regulations and maintain accurate medical documentation for legal and billing purposes. Requirements Valid RN license in good standing with the appropriate state licensing board. Proven experience working in diverse clinical environments such as hospital medicine, emergency medicine, or outpatient clinics. Strong knowledge of medical terminology, anatomy/physiology and electronic medical records (EMR/EHR systems) like PCC. Critical care experience preferred, including ICU or Level I/II trauma center exposure; experience with trauma medicine or emergency room procedures is highly valued. Skilled in advanced nursing procedures such as IV insertion, venipuncture, catheterization (urinary or vascular), tube feeding management, sterile processing techniques, and aseptic technique. Ability to handle complex patient cases involving infectious disease care, acute pain management, hospice & palliative medicine, occupational health issues including Workers' compensation law understanding. Excellent assessment skills with the ability to perform triage in urgent care or emergency settings; familiarity with laboratory experience is a plus. Experience working with individuals with disabilities or developmental delays in settings like assisted living or memory care is desirable.
