We are seeking an experienced RN Charge Nurse who values hands-on care, strong teamwork, and a manageable nurse-to-patient ratio. As the RN Charge Nurse, you will play a key leadership role on shift while still staying connected to resident care. This is an ideal position for an RN who enjoys autonomy, collaboration, and providing high-quality, patient-centered care in a close-knit setting. Why work at Oak Grove Manor? Small 11- bed facility with supportive, team-oriented environment Lower nurse-to-patient ratios compared to large SNFs Worked alongside experienced LVNs, CNAs, and support staff Leadership that values communication, respect, and clinical judgement Opportunity to truly know your residents and make a daily impact Schedule Requirements 12-hour shifts (AM and/or PM) Per Diem or Part-time availability Some weekend availability for part-time staff Responsibilities Serve as Charge Nurse during assigned shifts Provide direct nursing care based on individualized care plans Perform resident assessments and monitor changes in condition Supervise and support LVNs and CNAs on shift Administer medications and treatments safely and accurately Coordinate and complete admissions and discharges Communicate with physicians and interdisciplinary team members Ensure timely, accurate documentation in PointClickCare Respond promptly to clinical issues and emergencies Requirements Active California RN license in good standing Minimum 1 year of SNF or Subacute experience Curent BLS Certification Experience with PointClickCare Strong leadership, clinical judgement, and decision making skills Ability to work independently and manage shift responsibilities Trach and Vent experience How To Apply Please submit your application through indeed only. Pay:
