Field Registered Nurse - Home Health

Job Summary We are seeking a dedicated and experienced Field Registered Nurse to provide comprehensive home health care services. The ideal candidate will possess a strong clinical background, including experience with diverse patient populations such as geriatrics, and individuals with disabilities. This role involves delivering personalized nursing care in patients' homes, coordinating treatment plans, and ensuring compliance with healthcare standards and regulations. The successful applicant will demonstrate proficiency in electronic health record systems and a thorough understanding of medical terminology, diagnostic evaluation, and patient assessment. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and physiological evaluations. Administer medications, injections, IV infusions, and perform catheter care following aseptic techniques. Manage airway care, ventilator support, tube feeding, ostomy care, and wound management such as suturing. Document all patient interactions accurately using EMR/EHR systems Coordinate discharge planning and case management to ensure seamless transition of care across settings. Provide health coaching and education tailored to individual patient needs, including chronic disease management such as diabetes or cardiac conditions. Ensure infection control protocols are strictly followed to maintain a safe environment for patients and staff. Maintain compliance with HIPAA regulations and ensure confidentiality of all medical records and documentation. R equirements Active CA RN license Home health experience preferred Reliable transportation
